State Issues RFP for Survivorship, Palliative and Hospice Care, and Evaluation Initiatives (9/12/06)
The Connecticut Department of Public Health has
issued a Request for Proposals for initiatives in the areas of Survivorship,
Palliative and Hospice Care, and Evaluation. The RFP represents opportunities to
implement priority strategies pertaining to objectives in the 2005-2008
Connecticut Comprehensive Cancer Control Plan. Applicants may requests funds for
one or more of the initiatives.
A total of $675,000 is available for the projects: $180,000 for Evaluation,
$245,000 for a Survivorship needs assessment and inventory of resources, and
$250,000 for a Palliative and Hospice Care needs assessment and related
initiatives. Funding will be for 18 months, beginning January 1, 2008.
